RECORDS REQUEST FORM

Access to criminal reports/complaints maintained by the Otsego County Sheriff's Office will be in accordance with provisions of the Freedom of Information Law as set forth by Article 6 of the New York State Public Officers Law.

                   ALL REQUESTS MUST BE IN WRITING

Record Check: A $5.00 fee applies. FORM

Record Check : a check of convictions that are the result of an arrest made by the OCSO only.

Record checks and employment checks are done daily. You must appear in person with some form of I.D. and a letter will be typed and given to you while you wait.

** EMPLOYERS: Record checks can be done for you with a signed release. Information that needs to be included in this release: name, date of birth, social security number, address, signature, date and statement acknowledging that the release of criminal history information will be granted. You as a perspective employer will be responsible for the $5.00 fee.
