By registering for a clinic you are attesting that you meet eligibility criteria determined by New York State.
If you attest that you meet eligibility and you do not, you may be subject to a fine. Please read ALL eligibilty criteria.
REGISTRATION IS MANDATORY FOR ALL CLINICS. WE CANNOT ACCOMODATE WALK-INS. WHEN ARRIVING FOR CLINIC, PLEASE BRING A COPY OF REGISTRATION CONFIRMATION AS WELL AS IDENTIFICATION AND PROOF OF ELIGIBILITY.
OCDOH is able to vaccinate people that meet the following eligibility:
- First responders-firefighters, police, 911 workers
- State Field Investigators- DMV, SCOC, Justice Center, DFS, IG, Tax, OCFS, SLA
- Other Sworn & civilian personnel-court officers, support or civilian staff for above agencies
- Corrections-State and local
- Probation
- State & local juvenile detention and rehabilitation facilities
- P-12 schools-faculty, staff, contracted workers, bus drivers
- Day care personnel
- In-person college faculty and instructors
- public transit workers
- Public facing grocery store workers
- Individuals living or working in homeless shelters
- Individuals with following medical condition
Chronic kidney disease
Pulmonary Disease- such as COPD, asthma, pulmonary fibrosis, cystic fibrosis
Intellectual & Developmental Disabilities
Heart conditions- such as heart failure, coronary artery disease, cardiomyopathies, or high blood pressure
Immunocompromised state organ transplant, blood/bone marrow transplant, HIV, use of corticosteriods
Severe obesity/Obesity-BMI >30
Pregnancy
Sickle cell disease/Thalassemia
Type 1 or 2 Diabetes
Cerebrovascular disease
Neurological conditions-dementia, Alzheimer's Disease
Liver disease
*****If you are receiving a vaccine due to one of the chronic conditions listed above you do NOT need to show proof of eligibilty. You MUST attest to having the condition when you register*******
You may be eligible under other criteria, we can only vaccinate those listed above at health department clinics. If you meet criteria to be vaccinated under a category not listed please reach out to your local hospital.
If you are 65+ and have no chronic medical condition please click on the link below to see where you may be able to find a clinic
Individuals 65 years and older are eligible to be vaccinated. Per State DOH directives the Otsego County Department of Health cannot vaccinate individuals 65+.
If yre 65+ and received your first doseof vaccine at health department clinic you will receive your second dose at the same place.
